When you visit your Delta Dental provider, just give them the Group Number, which is located on the enrollment form (no ID card is issued). If you would like to have an ID card, go to www.deltadentalwa.com and click on the tab at the top of the screen that says “ID Card,” and you can print one out. You can also use the website to find preferred providers. Their Customer Service number is 1-800-238-3107.

Please note: In order to enroll for dental coverage, you must be enrolled for medical as well. Members who are not enrolled in the group-sponsored medical Plan may not enroll in the dental program.

Medical and Vision are not bound to another line of coverage – one can enroll in medical or vision separately or together.
