Welcome to Health Benefits! If you are interested in obtaining information on how to enroll in medical, dental, or vision benefits, you will be able to find all the information you need on this site. The links at the bottom of this page will enable you to download enrollment forms, benefit summaries, and rates for Group Health, Vision Service Plan, and Delta Dental.
Eligibility
In order to participate in the benefits available through the King County Bar Association, and you are an attorney you must be a KCBA member attorney or a member of an attorney group. If an employee other than an attorney, such as an assistant, secretary, etc., is seeking coverage, 80% of the attorneys in the firm must be a member of the KCBA. Everyone must also work a minimum of 20 hours a week in order to be eligible for coverage.
If you are a new member of the Bar, the earliest you would be able to enroll in one of the lines of coverage (medical, dental, and/or vision) is the first of each month. If you did not elect to enroll in one of the lines of coverage (medical, dental, and/or vision) at the time of becoming a member of the Bar, you can only enroll if you have had a qualifying event, or during open enrollment which is in June of every year with an effective date of July 1 through June 30 of the following year. A qualifying event is one of the following: termination of employment, reduction of hours which results in a loss of coverage, death of covered employee, divorce or legal separation, a covered employee’s eligibility for Medicare, a dependent child’s loss of dependent status, or loss of coverage due to the employees filing of a bankruptcy proceeding.
Please note: In order to enroll for dental coverage, you must be enrolled for medical as well. Members who are not enrolled in the group-sponsored medical Plan may not enroll in the dental program.
Medical and Vision are not bound to another line of coverage – one can enroll in medical or vision separately or together.
Guarantee Issue
The King County Bar Association plans continue to be guarantee issue which means you cannot be declined enrollment in the plan due to past or present health issues.
Rates and Plan Designs
The King County Bar Association rating continues to be group rated; however, the medical plans are on an age rated basis.
Billing
If you are the only one applying for insurance, the forms need to be received prior to the 1st of the month you wish to have your insurance start. For example, if you would like to start your coverage on July 1st, your forms need to be received by Wells Fargo Insurance Services by June 15th at the latest. To ensure you’re in the insurance company’s system by the 1st of the month for eligibility purposes and to have an ID card issued, it’s best to have your applications in at least two weeks before the beginning of the month.
If you are a new group (two or more employees), please submit forms to Wells Fargo Insurance Services on or before the 15th of the month prior to the month you would like to begin your insurance coverage. For example, if you would like to start your insurance coverage July 1st, the forms need to be received by Wells Fargo Insurance Services by June 15th.
When you’re ready to mail the enrollment forms back, please include your first month’s premium made out to KCBA (this binds your coverage) and mail them to:

All premium payments are due on the 25th of each month in order to bind your coverage for the next month and need to written out to KCBA. If payment is not received by the 25th, Delta Dental Service will terminate your coverage, and will not reinstate until premium is received.
